Mentorship Course Enrollment Form

We are thankful that God has given you a desire to help others as they come out of habitual sin through the power of the gospel. Amen! It is our hope and desire that this course will help you gain the ability to minister effectively to those who are enslaved to sin.

After completing the first 20 days of the mentorship program, you will be given the opportunity to apply to the staff of Setting Captives Free for a position as a mentor.

The prerequisites for working as a mentor with Setting Captives Free are listed below. Please examine them honestly and select your answers. Note: We respect the right of each person to believe as they choose, but Setting Captives Free will not accept enrollment requests from Muslims, Mormons, Jehovah's Witnesses, Catholics, Seventh-Day Adventists, Christian Scientists or Unitarians.

Do you understand that you will be ministering to others on a consistent basis and that, if you are going to be unavailable for a few days, you will need to notify your mentor coordinator in a timely manner? Yes No
